FAFSA Web Application
- This link informs students about the Free Application for Federal Student Aid process and allows students to apply on line. Students can also check the status of their federal application and view and/or print their student aid report.
HOPE Forms
HOPE Scholarship Application
(use menu bellow to select an appropriate application form)
I have not completed the GSFAPPS HOPE Scholarship Application at GAcollege411.org
You need to complete a GSFAPPS. If you have not already done so you must first create an account in GAcollege411.org
and be logged onto that website to complete the GSFAPPS. Once you have created an account you can login to GAcollege411.org.
Then:
1. Click on the tab at the top labeled "Applications & Transcripts"
2. Click on the link "GSFAPPS"
You must complete and transmit the GSFAPPS no later than the last day of the term for which you are seeking the HOPE
Scholarship in order for you to be considered for HOPE that term.
I have completed the GSFAPPS HOPE Scholarship Application at GAcollege411.org
Students who have completed the GSFAPPS HOPE Scholarship Application at GAcollege411.org generally do not need to reapply for
the HOPE Scholarship if they have been continuously enrolled and have received a disbursement of the HOPE Scholarship within
the last two semesters. If you have had a two semester break in enrollment or have lost your HOPE Scholarship eligibility for
two consecutive semesters, you need to complete a GSFAPPS.

HOPE Promise Application
- Use this application to apply for the HOPE Promise Scholarship Loan Program.
HOPE Teacher Application
- Use this application to apply for the HOPE Teacher Scholarship Loan Program.
HOPE Transient Form
- This form must be completed if you are a UGA student enrolling in classes at another HOPE eligible school. Please complete and submit to OSFA. You may want to make a copy for your records. Remember to keep track of the dates that academic transcripts must be received. Failure to comply may result in cancellation/repayment. Transient Non Enrollment Statement

Computer Expense Form 2006-07

Dependent Care Expenses Request Form 2006-07
- Please complete, sign, and return to OSFA. If you have questions, please call (706) 542-6147, press 1, and ask to speak to a Loan Counselor.
Low Income Verification Form (Dependent) 2006-07
- If directed to do so by OSFA, this form must be completed by parents who reported unusually low income on the 2006-07 Free Application for Federal Student Aid (FAFSA). If you have questions, please call (706) 542-6147, press 1, and ask to speak to a counselor.
Low Income Verification Form (Independent) 2006-07
- If directed to do so by OSFA, this form must be completed by independent students who reported unusually low income on the 2006-07 Free Application for Federal Student Aid (FAFSA). If you have questions, please call (706) 542-6147, press 1 and ask to speak to a counselor.
SAP Appeal Form
- Please complete, sign, attach required documentation and return to OSFA. You may want to make a copy for your records. Appeals are reviewed in the order in which they are received. If you have questions, please call (706) 542-6147, press 1, and ask to speak to a SAP counselor.
Scholarship Certification Statement 2006-07
- In order for OSFA to complete your Robert C. Byrd, Georgia HERO, Law Enforcement Dependent Grant, or Georgia Public Safety Memorial Grant application, this form must be completed, signed and returned. OSFA cannot process your application until this statement is received. If you have questions, please call (706) 542-6147, press 1, and ask to speak to a scholarship counselor.
Study Abroad Form
- This form is for use by students studying abroad who would like to document the additional costs associated with such programs. Students should contact OSFA to determine if such additional costs would result in increased eligibility before submitting this form. Verification Worksheet 2006-07
- If notified by OSFA, please complete the required sections, sign, and return to OSFA along with signed copies of your (and your parents' if required) 2004 Federal Income Tax Return and all 2004 W-2 forms. If you have questions, please call (706) 542-6147, press 1, and ask to speak to a counselor.
